Medical

Anthem Blue Cross and Blue Shield is the network administrator for the Centura Health Plus medical plan (Blue Preferred® - PPO Prime). You receive care from a large selection of participating healthcare providers at Centura facilities and other partnering physician offices. You pay no deductible and have an unlimited lifetime maximum benefit. In addition, a prescription drug program covers a wide range of prescription drugs. back to list

Dental
CIGNA Dental offers three choices of dental plans to meet the needs of you and your family: the preventive plan, the preferred plan and the dental HMO plan.

Vision
Vision Service Plan (VSP) offers comprehensive vision coverage for exams and vision wear. While you may use a vision care provider of your choice, you will receive greater benefits at a lower cost if you use member doctors.back to list

Retirement
Centura Health recognizes the importance of planning for your retirement and provides you with options to help build your retirement income.

Centura will contribute to your retirement account based on your years of service and annual earnings. You are eligible to participate on January 1 after you have completed and have been paid for 1,000 eligible hours of service in one calendar year.

You can choose to have Centura direct the contributions, or you may direct a portion of the contributions. You can also voluntarily contribute your own money to a tax deferred annuity (TDA) account (403(b)) with a portion of each paycheck. You determine how much to contribute, and you may increase or decrease your contribution at any time. Eligiblity in this plan occurs on the first day of your employment with Centura.

If you choose to direct your contributions in the retirement account and participate in the TDA account, Great West Life will provide you with a choice of investment approaches for your accounts. back to list

Life Insurance
Centura Health offers life insurance coverage for you and your dependents. Eligible associates automatically receive basic life insurance at no cost and have the option to purchase supplemental insurance for themselves and their eligible dependents. back to list

Disability Benefits
Long-term disability offers you and your family a partial continuation of income if you become disabled and are unable to return to work for an extended period of time. Full-time associates receive the long-term disability plan at no cost, while part-time associates can purchase the plan. A voluntary short-term disability plan provides partial continuation of income if you become temporarily disabled. back to list

Flexible Spending Accounts
Centura Health offers healthcare and dependent care flexible spending accounts (FSAs) to allow you to voluntarily set aside money on a pre-tax basis to reimburse yourself for eligible healthcare and dependent day care expenses. back to list

Employee Assistance Program (EAP)
The Employee Assistance Program (EAP) is a free confidential counseling service provided to you and your loved ones to help you deal with issues or problems that could potentially affect your health, relationships or job performance. You and other eligible dependents may receive up to five free sessions per issue per year. Services include counseling services, telephone legal service and financial and debt counseling services. back to list

Paid Time Off
Centura offers a paid time off program that includes vacation, sick time, and holiday pay. In addition to the Paid Time Off program, Centura offers an Extended Illness Bank in the event you become injured or ill and require an extended period of time away from work. back to list
